Flag football is a physical, contact sport. Please read this waiver carefully. By registering as a Team Captain or by joining a registered team, you are agreeing to the terms below and confirming you meet the league’s minimum age requirement of 21.
1. Acknowledgment of Risk
I understand that participation in Pacific Flag Football league play, practices, and related events (“the Activities”) carries inherent risks of physical injury, including but not limited to sprains, strains, fractures, concussions, collisions with other participants, and in rare cases, catastrophic injury or death. These risks exist even when rules are followed and reasonable precautions are taken. I voluntarily choose to participate with full knowledge of these risks.
2. Physical Condition and Medical Fitness
I represent that I am physically fit to participate in the Activities and have no medical condition that would make participation unsafe. I understand it is my responsibility to consult a physician prior to participating if I have any doubt about my fitness to play. I agree to notify league staff of any relevant medical condition prior to play.
3. Release of Liability
In consideration for being permitted to participate in the Activities, I, on behalf of myself and my heirs, executors, and assigns, release, waive, discharge, and covenant not to sue Pacific Flag Football, its owners, organizers, coaches, referees, volunteers, sponsors, and the venue (collectively, the “Released Parties”) from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in the Activities, whether caused by the negligence of the Released Parties or otherwise, to the fullest extent permitted by California law. This release does not apply to claims arising from the gross negligence, recklessness, or intentional misconduct of the Released Parties, to the extent such a limitation is required by law.
4. Assumption of Risk
I expressly assume all risks of participation in the Activities, whether known or unknown, and understand that this assumption of risk is a material term of my registration.
5. Indemnification
I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur due to my presence at or participation in the Activities, whether caused by my negligence or otherwise.
6. Medical Treatment Authorization
In the event of injury, I authorize Pacific Flag Football staff to secure emergency medical treatment on my behalf, and I agree to be financially responsible for any costs associated with such treatment that are not covered by my own insurance. Pacific Flag Football does not provide medical or accident insurance for participants.
7. Age Confirmation (21+)
I confirm that I am 21 years of age or older as of the first scheduled game date of the season I am registering for. Pacific Flag Football reserves the right to request proof of age.
8. Code of Conduct
I agree to conduct myself in a respectful and sportsmanlike manner toward players, officials, staff, and spectators, and understand that Pacific Flag Football may remove me from the league without refund for conduct it deems unsafe, unsportsmanlike, or in violation of league rules.
9. Photo and Video Release
I grant Pacific Flag Football permission to photograph and video me during the Activities and to use those images for promotional purposes (website, social media, marketing), without compensation. Participants who do not wish to be photographed may notify league staff in writing prior to the season.

11. Governing Law
This agreement is governed by the laws of the State of California. Any dispute arising from this waiver or the Activities shall be resolved in the state or federal courts located in Los Angeles County, California.
12. Electronic Signature
By checking the agreement box on the registration form and typing my full legal name, I am electronically signing this waiver and acknowledge it has the same legal effect as a handwritten signature. The system will record my name, email, and the date/time of my agreement as my signature record.
